Barry D. Forman

1947-2004

Barry D. Forman, 57, of 1200 Ashland Ave., Ashland, Ky., died Sunday, April 18, 2004, in King's Daughters Medical Center.

Mr. Forman was born Feb. 24, 1947, in Lexington, Ky., a son of Douglas G. Forman Sr. of Morehead, Ky., and the late Gloria Flanery Forman.

He was a construction supervisor with Hensel Phelps, was a member of South Ashland United Methodist Church and was a member of the U.S. Army Reserve serving in Vietnam and the Gulf War of 1991.

Additional survivors include his wife, Mary Lou Calvert Forman of Ashland; a daughter, Lesley Ellen Forman Lewis and her husband, Tom, of Morehead, Ky.; a son, James David Forman and his wife, Teresa, of Olive Hill, Ky.; his father and mother-in-law, George W. Calvert Sr. and Lucille Hamilton Calvert of London, Ky.; a brother, Douglas G. Forman Jr. and his wife, Gay, of West Liberty, Ky.; a sister, Sandra Forman Williams and her husband, Burl, of Morehead; and two grandchildren, Christian Lewis and Grace Lewis.

The funeral will be at 1 p.m. Tuesday, April 20, 2004, at South Ashland United Methodist Church in Ashland by the Rev. David Fultz, Tom Lewis and Kyle Crager. Burial will be in Brown Family Cemetery in Morehead.

Friends may call from 6 to 9 tonight, April 19, 2004, at Steen Funeral Home-Central Avenue Chapel.

There will be a military service conducted in Morehead.

In lieu lieu of flowers, contributions may be made to Memorial Rose Garden, c/o South Ashland United Methodist Church, 2203 29th St., Ashland, KY 41101 or Ashland ACTC Scholarship Fund, 1400 College Drive, Ashland, KY 41101.

Pallbearers will be Burl Williams, George W. Calvert Jr., Jack Hickey, Randy Johnson, Larry Grant and Kyle Crager.
