Hazel Mae Smith Click for Condolences      Printable Version 1960 - 2005 Hazel Mae Smith age 44 of Louisa, Ky. went to be with the Lord on Monday August 1, 2005. Hazel was born August 10, 1960 at Hode, Ky. the daughter of the late William Harvey and Barbara Ellen Mollette Blackburn. She was a homemaker and member of the Belles Chapel Free Will Baptist Church. Survivors include: Her husband: Paul Smith 4 sons: Ricky Smith, Jeremy Smith, Kyle Harmon, and Ralph "Scamp" Harmon A Sister: Carol Osborne 4 Brothers: Harvey Blackburn, Tarkey R. Blackburn, Glenn Darrell Hall, and Edmond Eugene Hall A Adoptive Sister: Rita Howard Funeral services were conducted on Thursday August 4th at 4:00 pm at the Belles Chapel Church by Rev. Rick Cordial. Burial followed in the Smith-Blackburn Cemetery at Louisa, Ky.
